- Monday - Friday, 8 am - 5 pm -- 9-hour shifts, 8 hours guaranteed
- 4 - 6 patients per shift
- Combination of in-clinic and community home visits required
- Adults over age 55 served through PACE program
- Geriatric experience required
- Experience working with interdisciplinary teams preferred
- Will consider 3 - 4 days per week schedule
