- Covers 3 locations per day -- approx. 6 locations per week
- Total patient panel of 340 patients across 8 locations
- Mix of short-term and long-term residents ages 55 and older
- New patient admissions with 30, 60, and 90-day follow-ups
- Part of a Patient Aligned Care Team (PACT) model
- Chronic disease management required -- HTN, DM, CHF, COPD, CKD
- GI management required -- GERD, constipation, malnutrition, incontinence
- Geriatric post-acute care experience required

From $70.00 to $95.00 hourly
Ranges shown should be used as an estimate and are affected by many factors including the critical need of the position, your overall experience and qualifications, and other considerations.
