Job Description
The primary focus of the Psychiatric Nurse Practitioner is to provide home-based medical care to chronically ill patients.
Details:
- Full-time, permanent
- Perform an initial comprehensive assessment on all newly enrolled members
- Develop a member management care plan upon enrollment
- Establish and maintain effective relationships with members and their legal representatives, staff, PCP’s, and other collaborating mental health providers
- Escalate and communicate any issues with severely mentally ill, complex mental health patients, and any active inpatient psych admission
- Understand and comply with long-term care policies and procedures
- Actively participate in weekly multi-disciplinary team meetings

Qualifications
- Current, unrestricted California NP license
- Completion of RN accredited program
- Completion of accredited program in Psychiatric/Mental Health Nurse Practitioner
- Minimum two years clinical experience
- Knowledge of CPT, ICD-9, and HCPC codes
