We bring comprehensive primary care directly to seniors' homes in assisted living and senior living communities across Arizona, California, and Nevada — eliminating barriers to care and helping them avoid unnecessary hospitalizations.

We're hiring Nurse Practitioners (Full-Time) to transform primary care in the East Bay area.


The Opportunity

Ready to practice medicine differently? Trade the 15-minute appointment treadmill for meaningful patient relationships and true clinical autonomy. You'll provide comprehensive, unhurried primary care to seniors with dedicated clinical support, smaller patient panels, and the flexibility to actually know your patients.

Our model is built around giving you time to practice medicine. Care coordinators handle administrative work, RNs manage triage, and you focus on excellent clinical care. You'll have robust team support for chart prep, patient education, and follow-up—creating a workflow that enhances both your satisfaction and patient outcomes.


What You'll Do

  • Deliver comprehensive onsite primary care to seniors in senior living communities
  • Develop personalized, patient-centered care plans
  • Partner with our Medical Director to shape clinical protocols and drive innovation
  • Provide monthly Chronic Care Management (CCM) visits
  • Collaborate with senior living leadership to optimize care execution and patient outcomes

What We're Looking For

  • NP certification with genuine passion for geriatric medicine (prior geriatric experience preferred)
  • Adaptable and energized by building something meaningful from the ground up
  • Comfortable providing onsite care across multiple communities
  • Mission-driven commitment to serving complex care patients
  • Strong communicator and team player who values collaboration
  • Tech-comfortable and flexible with scheduling/travel between communities
