The Opportunity
The Clinical Director (NP) at (Login for Information) combines clinical excellence with inspiring leadership. This hybrid role balances direct patient care with provider management—an opportunity to deliver exceptional geriatric care while developing and leading a talented team of 6-8 NPs and PAs in our innovative in-home care model.
You'll maintain meaningful clinical practice while focusing on clinical leadership, mentorship, quality improvement, and community partnerships. It's the rare opportunity to step into leadership without stepping away from patient care.
What You'll Do
Clinical:
- Deliver comprehensive onsite primary care with Welcome Visits, care planning, AWVs, and CCM visits
- Provide clinical coverage during provider absences
- Manage medication, assessments, and urgent behavioral health concerns
- Conduct post-discharge visits for transitioning patients
Leadership:
- Lead and develop a team of 6-8 NPs/PAs through performance reviews, mentorship, and professional development
- Optimize provider schedules and ensure metrics compliance
- Drive community growth and facility partnerships
- Lead quality improvement initiatives
- Hire, onboard, and train new clinical team members
What We're Looking For
- Active CA APRN license; California 103 NP license preferred
- Active unrestricted DEA
- Master's degree in Nursing with Family or Adult-Gerontology specialization preferred
- Minimum 3 years specialized geriatric experience in clinical, community-based, or senior living settings
- Minimum 3 years direct people management experience supervising clinical staff (performance reviews, metrics management, hiring decisions)
- Exceptional clinical acumen and diagnostic skills
- Ability to work autonomously in fast-paced, multi-site environments
- Must be able to drive between partner facilities regularly
