Internal Medicine - NP Other job located in California

We’re hiring an Adult/Geriatric Nurse Practitioner to provide home-based acute and transitional care across the Southeast L.A. area. This field role focuses on caring for patients post-hospital or post-SNF discharge, as well as medically complex, homebound adults.

Role Highlights

• In-home assessments, treatment, and care planning
• Manage acute and post-acute conditions
• Collaborate with physicians and care teams
• Educate patients and families to support recovery at home

Coverage Area
Southeast L.A. (including Paramount, Norwalk, Huntington Park, Bellflower, and surrounding areas)

Schedule & Pay

• Full-time, Monday–Friday
• $119K–$149K base salary + bonus eligibility

Requirements

• MSN + Adult/Geriatric NP certification
• Active NP/RN license
• 1+ year NP experience (home-based care a plus)
