Heart Failure Advanced Practice Provider (APP)
Position Summary
The Heart Failure Advanced Practice Provider (APP) will provide comprehensive care for patients with heart failure as University Health builds its advanced heart failure and mechanical circulatory support program. The APP will collaborate with cardiologists, registered nurses, and other members of the multidisciplinary team to deliver guideline-directed medical therapy (GDMT), optimize transitions of care, and improve quality outcomes for patients with heart failure across the continuum.
Essential Responsibilities
Patient Care
• Conduct new patient consultations and follow up evaluations for heart failure patients.
• Rotate on rounding on inpatient heart failure consults
• Manage initiation and titration of GDMT in collaboration with supervising physicians.
• Provide education and counseling to patients and families regarding disease management, medications, lifestyle modifications, and advanced therapies.
• Partners with heart failure nurse navigators and coordinators to support early post-discharge follow-up and prevent avoidable readmissions.
• Facilitate integration of telemedicine and remote monitoring in HF management.
Program Development & Quality
• Assist in the development of protocols and workflows to improve patient outcomes, safety, and efficiency.
• Contribute to heart failure quality initiatives, including reducing readmission rates and improving adherence to guideline-directed therapies.
Education & Leadership
• Serve as a clinical resource for HF nurse navigators, APP colleagues, and nursing staff.
• Provide ongoing education to patients, caregivers, and the healthcare team on HF disease management and advanced therapies.
Qualifications
• Nurse Practitioner (NP) or Physician Assistant (PA) with current Texas licensure (or eligibility for licensure).
• Master’s degree required; Doctorate preferred for NP candidates.
• Minimum 2–3 years of experience in cardiology, heart failure, or internal medicine required (HF specialty experience strongly preferred).
• Strong knowledge of GDMT for heart failure and familiarity with advanced HF therapies.
Excellent clinical judgment, communication, and patient education skills.
• Demonstrated ability to work collaboratively in a multidisciplinary team.
