Nurse Practitioner Elkhart, IN (15 miles E of South Bend)

We are looking for a motivated Nurse Practitioner to join our medical practice in Elkhart, IN. We are looking for a provider who can join us providing direct patient care. Our ideal NP is holistically minded with an interest in functional medicine, comfortable working within an integrated setting, collaborating with a knowledgeable and well-respected team of professionals, to offer a comprehensive approach to patient’s health and wellness concerns. The incoming NP will be responsible for delivering excellent patient care under the supervision of a collaborating physician with a focus in clinical procedures, hormone therapy, and administering IV infusions.

Duties:

• Patient exam/evaluation (medical history, physical exam, review pertinent med records/imaging)
• Order and review tests/labs
• Diagnosis and treatment plan formulation
• Patient education as to diagnosis, treatment options, rehab/exercises and various modalities
• Manage patient care including hormone replacement
• Treatments- perform trigger point and joint/facet injections, administering IV infusions
• Documentation and progress notes
• Collaboration with the multidisciplinary team for comprehensive patient care

Requirements:

• Graduation from an accredited NP program
• NP license in IN
• Orthopedic, Physical Med and Rehab, or Pain management background is a plus
• Experience with injections and IV infusions a plus
• 2+ years of clinical work experience (Preferred)
• Interest in Holistic and Functional Medicine

Compensation:

• $120k
